JOB SUMMARY
:
The Anticoagulation Clinical Staff are responsible for the Anticoagulation Clinic Service.
! The pharmacist assists prescribers in caring for patients on anticoagulation therapy to achieve desired outcomes.
QUALIFICATIONS:
PharmD preferred. A
t a minimum, a BS in Pharmacy, or
Iowa Pharmacist Licensure, either by examination or by reciprocity.
Completes Mandatory Reporting requirement within 6 months of hire and renews this requirement every 5 years.
Demonstrated competence in providing direct patient care.
Previous anticoagulation management experience preferred.
Good communication skills.
Leadership skills.
