The VA Central Iowa Health Care System provides a full continuum of care between the Des Moines campus and our Community Based Outpatient Clinics (CBOCÂ's) and is one of the Medical Centers that make up the VA Midwest Health Care Network

(VISN 23).

DUTIES:
THE SALARY FOR THIS POSITION WILL BE DETERMINED BY A PROFESSIONAL STANDARDS BOARD AND WILL BE BASED ON EDUCATION AND EXPERIENCE LEVELS. Salary range is $91,467-$126,042

Work Schedule:
Rotating shifts:
7am-330pm; 8am-430pm; 12pm-8pm during the week; 7am-3pm weekends and holidays (weekends and holidays are rotating)

DUTIES:
The scope of practice of the Pharmacist covers direct and indirect patien! t care, drug therapy monitoring, prescriptive authority, quality assurance/improvement, staff/patient education and consultation. The pharmacist serves a multi-functional role and is expected to have an advanced knowledge of pharmacology, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.

Inpatient Pharmacist

: Activities included in the daily function of the inpatient pharmacist included but are not limited to: Order entry, admission assessments, transfer assessments, identification and entry of adverse drug reactions into the system, review of patients on anticoagulation therapy, discharge counseling, IV to oral conversions as listed by protocol, review of therapy and recommendations to providers as appropriate, recommend renal dosing for therapy as appropriate, admix IVs, adhere to USP 797 criteria, serve as a resource for patients and healthcare providers, comply with pharmacy policies and procedures, participate in advancement of pharmaceut! ical care for patients. Inpatient pharmacists may function in the follo! wing outpatient areas: Filling area and Clinics. Inpatient pharmacists may cover an area if desired and it is part of their scope of practice (e.g. Anticoagulation). The inpatient pharmacists participate in the pharmacistÂ's on-call schedule. The pharmacist must be able to be on site within 60 minutes of receiving a call. Assignments include pharmacist duties, tasks, and responsibilities across pharmacy services.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:

U.S. citizenship.

·

Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

·

Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.(

Exception.

Non-licensed phar! macists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of

38 U.S.C. § 7405(c)(2)(B).

The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist.

For grade levels above the GS-11, the candidate must be licensed.)

GRADE DETERMINATIONS:
GS-11 Pharmacist -

! None beyond the basic requirements.

GS-12 Clinical Pharmacist (Full Performance Level)-

1 year of experience equivalent to the next lower grade level,

or

completion of an ACPE-accredited Pharm.D. program.
