The VA Sierra Pacific Network (VISN 21) is one of the twenty-one Veterans Integrated Service Networks (VISNs) within the Veterans Health Administration (VHA). VISN 21 is a large and geographically diverse region, spanning 240,000 square miles and includes Hawaii, the Philippines, and Island of Guam, northern and central California, and northern Nevada. VISN 21 includes six medical centers and over 30 community based outpatient clinics. The Network health care systems and their clinics vary in complexity and include general medical and surgical facilities, large outpatient centers with VA/DoD sharing arrangements, nursing home facilities, outpatient care, domiciliary, and highly affiliated tertiary care centers.

The incumbent of this position is responsible to provide specialized support to develop, maintain, and educate clinical staff on safety and performance programs for VISN 21. Projects may include Clinical Practice Guidelines tracked by the PBM Data Warehouse, PBM performance measures, Lost Opportunity Costs (LOC)/PBM contract adherence and other pharmacoeconomic initiatives. In addition, the incumbent will participate in the development of reports providing comparative data on clinician panels at VISN 21 medical centers. The incumbent will consult with VISN and PBM staff to help initiate, develop, validate and execute projects with support of the data warehouse. The incumbent will be expected to exercise initiative and to collaborate effectively with clinical and administrative staff at all VISN stations to meet performance goals and support quality patients care activities.

Possible duty locations are Washington, Oregon, Idaho, California, Nevada, Utah, Colorado, Wyoming, Montana, Arizona, New Mexico, and Texas.

DUTIES:
Program Management

The incumbent acts as a knowledge resource to VISN 21 staff, including the PBM Managers, Quality Managers, Pharmacoeconomic Pharmacists, clinicians and administrators regarding data warehouse projects of the VISN 21 PBM. The incumbent will seek clinician input into the design of projects. The incumbent must be able validate the structure and function of information management systems using ProClarity Desktop Professional and Microsoft Business Intelligence Suite (Microsoft SQL Server 2008, Microsoft Sharepoint 2010, Performance Point 2007). The incumbent must be able to recommend appropriate implementation strategies for planned activities taking into consideration the clinician using the tool as well as understanding the technical development limitations of the software and staff.

The incumbent will retrieve best practice information from model sites and implement efforts for quality management. He/she will coordinate targeted physician and pharmacist task forces to address performance measures, and optimize the cost-effective use of medications and respective concerns.

Develop and implement VISN-wide targeted programs to achieve performance measures and contract goals in a global and time-efficient manner. He/she will monitor needs of individual sites and educate sites on project implementation.

Generates reports, searches, and lists for VISN 21 PBM and other VA personnel as appropriate for research and program purposes. Use of Proclarity Desktop Professional, as well as a working knowledge of Structured Query Language (SQL), and the Microsoft Business Intelligence platform or the management and analysis of data required with a knowledge of MDX code preferred.

Education/Training

Provide targeted physician, resident and pharmacist education to sites and providers based on target performance areas and data. Develop patient targeted education that will essentially enlist the patient to prompt providers to adhere to contract and performance measure goals as appropriate.

Work with station clinicians to identify, develop and implement target programs. Develops and performs formal and informal education to promote these programs and gain acceptance.

Participates in the precepting of pharmacy residents and students as part of duty responsibilities to increase the awareness and understanding of pharmacoeconomic and managed care issues.

Outcomes/Research

Monitor outcomes of programs, including cost-containment, healthcare resource utilization, and clinical patient information; provide feedback of successes for continued motivation and support for any necessary changes. Continually monitor new literature, evidence and pricing changes to improve care and cost-containment. Provide feedback, education, and recommendations to each site to improve quality of patient care and healthcare resource utilization.

Designs and participates in research studies involving therapy outcomes, pharmacoeconomics and quality care. Prepares manuscripts/abstracts for publication and presentation at professional meetings.

Full emphasis will be placed on making the VISN 21 PBM program a success and contributing to the successful and complete handling of pharmacy related projects. The incumbent is also expected to be an active participant in the VISN 21 PBM. He/she will work effectively with VISN 21 PBM staff, Quality Managers, clinicians and CPG champions. Every effort will be made to convey information to the local facility to improve outcomes consistent with performance initiatives. This will require coordination with different staff at each VISN 21 facility to accomplish goals.

EDUCATION REQUIREMENT:
(1) Candidate must be a graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP). (2) Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. This finding may be based on any of the following:

(a) A letter of acceptance into a United States graduate pharmacy program recognized by the ACPE.

(b) Written certification from the Foreign Pharmacy Graduate Examination Commission, 700 Busse Highway, Park Ridge, IL 60068, Phone (847) 698-6227, that the individual has successfully passed the Foreign Pharmacy Graduate Examination.

(c) A letter from a United States college or university with a pharmacy degree program recognized by ACPE stating that the individualÂ's pharmacy degree has been evaluated and found to be equivalent to its Bachelor of Pharmacy degree or higher.

LICENSURE REQUIREMENT:
Candidate must possess a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, is not eligible for appointment to the position unless such revoked or surrendered license is fully restored. 38 U.S.C. § 7402(f). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.

GRADE DETERMINATIONS:
GS-13 Program Manager: Individuals in these assignments coordinate a single program area of a complex pharmacy operation, or multiple programs areas within a facility, to develop, organize, manage, and control complex pharmacy programs. Examples include but are not limited to nuclear pharmacy, complex information technology ADPACs, Pharmacy Informatics, Clinical Application Coordinators (CACs) Specialist, Pharmacy Information Systems, nutritional support services, specialized IV services, oncology, pharmacokinetic consulting, ambulatory care, psychopharmacy, drug information, quality assurance/utilization review, spinal cord injury, pharmacoeconomics etc.)

SPECIALIZED EXPERIENCE REQUIREMENT:
Completion of the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled, or board certification recognized by the American Pharmaceutical AssociationÂ's Board of Pharmaceutical Specialties, or the American Society of Consultant Pharmacists (or equivalent), or completion of a two year post Pharm.D. fellowship in a clinical or specialized program.

Examples of creditable Specialized Experience includes any one of the following:

1. Clinical Pharmacist. A Clinical Pharmacist spends significant time in Clinical Practice (> 25 percent). Examples of practice sites include but are not limited to: medicine, surgery, psychiatry, neurology, spinal cord injury, primary care, ambulatory care including open pharmacy, rehabilitation medicine, geriatrics, medical research and others recognized by the professional standards board.

2. Pharmacy Specialist. Pharmacists in these assignments have responsibility for managing a specialized area of practice within a facility or area of a facility. They include but are not limited to IV admixtures, Information Technology (ADPACs, Informatics Specialists), Drug Information, etc. which require specialized skills but may be supervised by other pharmacists and have close relationships with other departments or services.

3. Pharmacy Supervisor, Inpatient/Outpatient. The supervisor (Inpatient/Outpatient) is responsible for the professional and administrative management of an inpatient/outpatient area in a small minimally complex pharmacy service that offers basic pharmacy services at a Complexity Level 3 (low complexity) VHA facilities with one division, or an inpatient/outpatient area at a site geographically removed from the supervisor. Pharmacy programs at these facilities are primarily involved in distributive functions and general clinical pharmacy activities. Such individuals typically have responsibility for supervising three or more professional GS-11 pharmacists. In addition to the experience requirements in paragraph (a), pharmacists assigned to these positions must demonstrate an ability to manage people and/or programs and the knowledge, skills, abilities and other characteristics necessary to satisfactorily complete the following duties.
