The VA Central Iowa Health Care System provides a full continuum of care between the Des Moines campus and our Community Based Outpatient Clinics (CBOCÂ's) and is one of the Medical Centers that make up the VA Midwest Health Care Network (VISN 23). Employee selected may be expected to perform duties at other duty sites within VA Central Iowa Health Care System based on the needs of the organization.

DUTIES:
THE SALARY FOR THIS POSITION WILL BE DETERMINED BY A PROFESSIONAL STANDARDS BOARD AND WILL BE BASED ON EDUCATION AND EXPERIENCE LEVELS. Salary range is $91,467-$126,042 .

Work Schedule:
7am-330pm; 8am-430pm; 830-5pm; 930am - 6pm during the week;

7am-3pm weekends and holidays (weekends and holidays are rotating)

DUTIES:
The scope of practice of the Pharmacist covers direct and indirect patient care, drug therapy monitoring, prescriptive authority, quality assurance/improvement, staff/patient education and consultation. The pharmacist serves a multi-functional role and is expected to have an advanced knowledge of pharmacology, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.

I npatient Pharmacist : Activities included in the daily function of the inpatient pharmacist included but are not limited to: Order entry, admission assessments, transfer assessments, identification and entry of adverse drug reactions into the system, review of patients on anticoagulation therapy, discharge counseling, IV to oral conversions as listed by protocol, review of therapy and recommendations to providers as appropriate, recommend renal dosing for therapy as appropriate, admix IVs, adhere to USP 797 criteria, serve as a resource for patients and healthcare providers, comply with pharmacy policies and procedures, participate in advancement of pharmaceutical care for patients. Inpatient pharmacists function in the following outpatient areas: Filling area, Clinics, and the Phone room. Inpatient pharmacists may cover an area if desired and it is part of their scope of practice (e.g. Pain Service, LTC). The inpatient pharmacists participate in the pharmacistÂ's on-call schedule. The pharmacist must be able to be on site within 60 minutes of receiving a call. Assignments include pharmacist duties, tasks, and responsibilities across pharmacy services.

Outpatient Pharmacist:
Activities included in the daily function of the outpatient pharmacist include but are not limited to: Order entry, admission assessments, transfer assessments, identification and entry of adverse drug reactions into the system, review of patients on anticoagulation therapy, IV to oral conversions as listed by protocol, review of therapy and recommendations to providers as appropriate, and recommend renal dosing for therapy as appropriate. Clinical activities include but are not limited to: Provides oral and written consultation with other health care professionals regarding drug therapy selection or management in an accurate, timely and professional manner; Utilizes the highest level of clinical privileges for their position to manage assigned patients. Interfaces with providers, including physicians, PAs, and ARNPs, in the design of the veteranÂ's treatment regimen to achieve optimization of rational and cost-effective drug therapy; Identifies all drug-induced problems that may be affecting the patient or contributing to a need for hospitalization. This would include drug side effects, allergic reactions, toxicities, improper drug regimens, non-compliance, etc. Recommendations are made accordingly to the responsible provider either verbally or in writing; Provides and promotes cost effective pharmacy services, offering reasonable, appropriate alternatives to high cost drugs; and Provides care and/or services appropriate to the age of the patients being served. Assesses data reflective of the patientÂ's status and interpret the information needed to identify each patientÂ's requirements relative to their age-specific needs and to provide care needed as described in the servicesÂ' policies and procedures; Serves as a reference source and guide to proper IV therapy and scientific compounding techniques for physicians, nurses, and other members of the health care team.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
U.S. citizenship. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.( Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed.) English Language Proficiency: Social workers must be proficient in spoken and written English. PHYSICAL REQUIREMENTS: In accordance with 29 Code of Federal Regulations 1614.203, the individual selected must be physically and mentally able, with or without reasonable accommodation, to perform the essential functions of this position without endangering the health and safety of self and others

GRADE DETERMINATIONS:
GS-11 Pharmacist - None beyond the basic requirements. ( NOTE: See exception to licensure requirement in subparagraph 2c(1) above.)

GS-12 Clinical Pharmacist (Full Performance Level)- 1 year of experience equivalent to the next lower grade level, or completion of an ACPE-accredited Pharm.D. program.
