PHYSICAL REQUIREMENTS: The work of
this position is primarily sedentary. Physical demands do not exceed those of a

The incumbent will be
involved in the independent professional and clinical practice of pharmacy in
accordance! with the guidance set forth in Pharmacy Service Policies and
Procedures, VA regulations, and state and federal laws governing the practice
of pharmacy. The Clinical Pharmacist will work in conjunction with other
pharmacy staff and other healthcare professionals to form an organized approach
in the delivery of pharmaceutical care. The Clinical Pharmacist will be under
the administrative supervision of the Outpatient Pharmacy Program Manager. The
incumbent may be scheduled and assigned to different areas of pharmacy practice
within Pharmacy Service. Principal duties include providing innovative and
progressive clinical pharmacy services for Outpatient Ambulatory Care Clinics,
Outpatient Specialty Clinics and/or Community-Based Outpatient Clinics
(CBOCÂ's).
The following is a brief listing

of additional duties of this position:
1.
Provides pharmacokinetic, pharmacodynamic, and pharmacotherapeutic consultation!
to the outpatient and inpatient professional staff. Evaluates and ! provides an
assessment of physiciansÂ' orders, side effects, appropriateness of route and
method of administration, and drug/drug, drug/food, drug/laboratory, and
drug/disease interactions.
2.
Obtains medication histories from patients, when appropriate, and summarizes
the significant findings. These findings are then documented in the medical
record and reported to the primary physician.
3.
Identifies any drug-induced problems that may be affecting the patient or
contributing to the need for hospitalization and promptly acts to achieve their
resolution.
4.
Analyzes the patientÂ's medication regimen for drug interactions and determines
the potential for and significance of each interaction. Incumbent acts to
achieve optimal management of drug interaction problems. Anticipates and
recognizes patient problems that are a result of or will influence their drug
therapy, and facilitates solutions when problems oc! cur.

5.
Additional duties as assigned.

SALARY
RANGE: $92,418 per annum - $110,499 per
annum. Your salary and grade will be determined by the Pharmacist Professional
Standards Board (PSB). The PSB is a peer review process. The PSB will review
your individual qualifications, education, professional training/internship(s)
and professional experience to determine the most appropriate salary and grade
based on your individual qualifications.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
U.S. citizenship.

Proficient in spoken and written English.

Full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States, or the District of Columbia.

Graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACP! E) or prior to the establishment of ACPE, have been a member of the Ame! rican Association of Colleges of Pharmacy (AACP).

Completion of 1 year of creditable experience as a Pharmacist at the GS-11 level or equivalent, OR board certification recognized by the American Pharmaceutical Association's Board of Pharmaceutical Specialties, or the American Society of Consultant Pharmacists (or equivalent), OR completion of a year post Pharm.D. fellowship in a clinical or specialized program or OR completion of a 1 year post-Pharm.D. ASHP accredited VA Residency within the next 90 days.
