Infusion Therapy Nurse

Location: Iowa City, Iowa

a. Be available by pager or phone during normal working hours, when on call,
and when on back up call.
P! hone and pager must be at a setting/mode that promptly alerts the staff member. If assigned pager or phone is not available staff must get a replacement. The staff member is responsible for any cost the company incurs due to lost equipment.

b. Admit patients to the UICH service including all aspects of the admission process including:
Entering patient information into CPR+ as requested.
Providing direct patient and/or family teaching for home infusion therapies prior to discharge.
Reviewing admission paperwork/forms with patient
Generating a Care Plan within 72 hours of admission

c. Performs patient visits according to Care Plan or as patient need dictates.

d. Delivers care and instruction that is appropriate to the patients age.

e. Performs required documentation including attempted point of care medical record documentation, inventory records and verbal and written communication with referral sources.

f. Coordin! ate referral process for infusion therapy services with pharma! cy, referral sources and subcontracted home care staff.

g. Communicate regularly with pharmacist regarding the specific instructions to and needs of UICH patients. Routine communication is to include:
patients clinical status
changes in status
changes in orders

h. Perform case management activities for assigned patients. Responsibilities for case management include:
Monitor progress of pharmacy, nursing, and DME patients. Information may be obtained from EPIC, CPR+ or direct patient contact.
Track the number of nursing visits/hours per week and notifying the Nurse Manager if additional hours required

i. Participate in the development, implementation, and evaluation of home infusion therapy education protocols and materials.

j. Participate in the orientation of new nursing staff.

k. Participate actively in UIHC quality assurance programs including:
Completion of unusual occurrence reports
Selecting appr! opriate choice in the CPR+ QA indicator
Participating in medical record audits
Participates in quarterly quality assurance meetings

l. Participate in nursing staff education and training programs and maintain skills for assessing and teaching home infusion therapy patients.

m. Participate in regular schedule and on-call schedule for infusion therapy services during evenings and weekends.

n. Maintain skills for delivering infusions and teaching home infusion therapy patients according to UICH Policies and Procedures and industry standards.*

o. Assist the Director and Nurse Manager in the development, implementation, and evaluation of marketing strategies for home infusion therapy services and attempt to identify potential home care candidates within the UIHC.

p. Respond to verbal and written issues, concerns, or complaints from home care patients, families, physicians, nurses, other UICHs staff, insurance companies, contra! ct vendors, or other outside vendors.

q. Adhere to UICHs Compl! iance Program and Code of Conduct

r. Attend annual mandatory in-services
Safety
Infection Control
Hazardous Communication
Four other inservices/year with two addressing pharmaceutical issues

s. Maintain CPR competency.
t. Maintain Reporting Dependent Adult and Child abuse training as required.
Perform other duties as assigned.

Physical Requirements
Drive a car daily, sometimes distances of 100 miles one-way.
Lift/push/pull up to 50 lbs.
Sit at a desk, working on a computer for periods ranging from 15 min to hours at a time.
5. Contacts
Health Insurance Companies: Occasional written and verbal communications regarding patient referrals and rationale for home infusion therapy and resolution of issues or complaints.

UIHC Staff: Multiple times per day, verbal and written contacts with UIHC staff supporting activities regarding services, delivery, availability, patient care delivery and! care coordination.

Pharmacy Staff: Daily contact concerning the compounding, schedule, and availability of medications. Provide updates on clinical issues to pharmacy staff.

6. Minimum qualifications

a. Associate Degree (minimum) in nursing and current RN license required.

b. Recent infusion therapy patient care services delivery and teaching experience, preferably in home health care or other alternate delivery systems.

c. Effective interpersonal and written and verbal communication skills.
