Position Summary: The primary responsibility of this position is to perform medication therapy management services to optimize the qua! lity of pharmaceutical care provided by our company, as well as to meet the patient’s personal healthcare goals. This includes chronic disease state management as well as facilitating transitions of care in coordination with other key stakeholders in our community.Responsibilities/Duties: Facilitate the formalization, growth and management of the following clinical programs: oChronic Disease Management (CDM) oMedication Therapy Management (MTM) oAdherence Management & Medication Reconciliation Identify patient need for participation in clinical programs, facilitate their enrollment.Provide updates and relevant information to our pharmacy department and managing providers to ensure patient safety and achievement of intended outcomes throughout their participation in our clinical programs.Oversee technician staff in the management of our adherence programs, including their patient outreach and dispensing activities.Serve as a reference for drug information primarily for our! pharmacist staff, and secondarily for the staff within the pr! imary care clinic in which we're located. Coordinate clinical outreach relevant to ambulatory care to local stakeholders.
